He came in at 34 with a Norwood 4 pattern that had been stable for about three years, and a donor area measured at 78 follicular units per cm². Stable loss and a strong donor is the easiest version of this operation, and it is why the plan was a single session rather than two.
3,200 grafts across the frontal third and mid-scalp, with the crown deliberately left alone. Covering the crown as well would have used another 1,500 grafts from a donor area that needs to last another thirty years. He agreed to leave it, which is the decision that makes this result still look right in a decade.
Drawn two centimetres above the original adolescent line, with an irregular front row of single hairs. A lower line would have photographed better at six months and worse at sixty.
Density at the front is where we expected. The crown has thinned slightly further, as predicted, and he started finasteride at month three to slow that. He is scheduled for a review at two years to decide whether a small crown session is worth it.
