Profile view six months after open rhinoplasty

She was explicit at the consultation: remove the hump, lift the tip slightly, and do not give her the small upturned nose she had seen on other patients. That is a reasonable brief and a surprisingly common one.

What was done

An open approach, dorsal hump reduction of about 4 mm, lateral osteotomies to close the open roof, and a columellar strut to support the tip. No alar base reduction, because narrowing the nostrils would have changed the character of the face more than the hump ever did.

Six months

Most of the swelling has gone, though the tip will continue to refine for another six to twelve months — that is normal and worth knowing before you judge a result at this stage. The profile is straight, the tip is supported, and the nose still reads as hers in family photographs, which was the brief.
