Surgery After a Previous Nose Operation

Revision rhinoplasty is for people who are unhappy with the appearance, the breathing, or both, after an earlier nasal operation. It is more complex than a first procedure because the anatomy has already been altered.

• Previous incisions, grafts and areas of collapse are mapped on examination.
• Operative notes from the first surgery help when they are available.
• A second operation is offered only if a defined problem can be addressed safely.

Scar Tissue and Safe Limits

Scar tissue is stiffer and less predictable than untouched cartilage. It limits how far the skin can redrape and how finely the tip can be reshaped. Not every previous result can be fully corrected.

• Thick scar can blunt definition even when the framework is improved.
• Cartilage may already have been removed, so new support may need to come from elsewhere.
• Some irregularities can be softened rather than erased.

Assessment Before Further Change

The consultation is as much about what cannot be done as about what can. Anaesthesia, graft choices and the chance of another revision are discussed before any date is set.

• You are told if waiting for further scar softening is safer than operating now.
• Follow-up is longer, because swollen, scarred skin settles slowly.
