Clarification Text of the Law on the Protection of Personal Data
This text is in the nature of an annex to the membership agreement and includes the clarification text regarding the Personal Data obtained with the contract and the express consent statement.
6698 sayılı Kişisel Verilerin Korunması Kanunu uyarınca VERİ SORUMLUSU şirketimiz ESTEHAİR SAÇ EKİMİ ESTETİK VE CERRAHİ HİZMT. A.Ş. (her biri “Şirket”), işbu Üyelik Formu / Bilgi Alma Formu sayesinde elde etmiş olduğu, üyelik hesabı açılmasına ilişkin başvuru talebini ileten kişiye ait kişisel verileri (Ad/Soyad/e-mail/Telefon numarası/Adres) ilgili mevzuatın izin verdiği durumlarda veri sorumlusu olarak kaydedebilir, sınıflandırabilir, işleyebilir, saklayabilir, güncelleyebilir ve üçüncü kişilere aktarabilir.

Üyelik hesabı / danışan kaydı açılmasına ilişkin başvuru talebini ileten kişinin a) kendisine ait kişisel verilerin işlenip işlenmediğini öğrenme, b) kişisel verileri işlenmişse buna ilişkin bilgi talep etme, c) kişisel verilerinin işlenme amacını ve bunların amacına uygun kullanılıp kullanılmadığını öğrenme, d) yurt içinde veya yurt dışında kişisel verilerinin aktarıldığı üçüncü kişileri bilme, e) kişisel verilerinin eksik veya yanlış işlenmiş olması hâlinde bunların düzeltilmesini isteme, f) mevzuatta öngörülen şartlar çerçevesinde kişisel verilerinin silinmesini veya yok edilmesini isteme, g) (e) ve (f) bentleri uyarınca yapılan işlemlerin, kişisel verilerinin aktarıldığı üçüncü kişilere bildirilmesini isteme, h) işlenen verilerinin münhasıran otomatik sistemler vasıtasıyla analiz edilmesi suretiyle kendisi aleyhine bir sonucun ortaya çıkmasına itiraz etme, ve i) kişisel verilerinin kanuna aykırı olarak işlenmesi sebebiyle zarara uğraması hâlinde zararın giderilmesini talep etme hakları bulunmaktadır.

Yukarıda belirtilen haklara ilişkin talepler, noter kanalı ile Harbiye, Vali Konağı Cd. No 12 / 2, 34367 Şişli/İstanbul adresine iletilmesini takiben en geç 30 gün içerisinde ücretsiz sonuçlandırılacaktır.

Yukarıda tarafıma yapılan bilgilendirme çerçevesinde, verdiğim tüm kişisel bilgileri kendi rızamla verdiğimi beyan eder, vermiş olduğum kişisel verilerin Şirket tarafından yukarıda belirtilen amaçlarla kullanılmasına, kaydedilmesine ve bu amaçlarla yurt içi ya da yurt dışına aktarılmasına, kişisel iletişim bilgilerimin bu rıza beyanı şartları dâhilinde saklanmasına, kaydedilmesine ve üçüncü kişiler ile paylaşılmasına açık muvafakatim olduğunu, kişisel verilerinin işlenmesine ve korunmasına ilişkin olarak bilgi talep etme hakkım dahil olmak üzere tüm haklarımı öğrendiğimi ve veri sorumlusuna başvurarak haklarımı Kanuna uygun biçimde her zaman kullanabileceğim noktasında bilgilendirildiğimi kabul ve beyan ederim.
